The notice of delinquency can be made to refer to the delinquent obligation
as an 'account' or a 'debt'. Enter one of these terms in response to this
prompt.
{ACCOUNT_NAME}
This entry is for the name of the account as shown on the records of the Bank.
{ACCOUNT_NUMBER}
Enter the account number for the account on which the check was drawn.
{ADDRESSEE_ADDR1}
Enter the first address line of the name and address field of the letter or
notice being prepared. This may be the name of a company, if appropriate.
{ADDRESSEE_ADDR2}
This entry is for the second address line of the name and address field. It
may be skipped for a three-line name and address field by entering two
returns <RETURN><RETURN>.
{ADDRESSEE_NAME}
This entry is the first line in a four-line name and address section of the
letter or notice being prepared. This line will usually be the name of the
person to whom the letter or notice is being directed.
{BALANCE_DUE}
Enter the dollar amount of the outstanding debt or account due including all
charges for which payment is being demanded.
{CHECK_AMOUNT}
This entry is for the total amount for which the check was made.
{CHECK_DATE}
Enter the date shown on the check as the date it was issued.
{CHECK_NUMBER}
Enter number of the check to assist Bank in its identification.
{CITY/STATE/ZIP}
This entry is for the City, State and Zip Code to be shown in the name and
address field of the letter or notice.

Enter the name of the lawyer's client for which this dunning letter is being
prepared.
{COLLATERAL_DESC}
You may include a paragraph of information describing the collateral, of
which delivery is sought.
{CREDITOR_NAME}
This entry is for the name of the creditor demanding payment.
{CURRENT_DATE}
This is for the date which will appear on the notice or letter being prepared.
{DEADLINE_DATE}
This entry is for date by which payment is demanded before other action will
be taken to collect the debt.
{DEBTOR_NAME}
Enter the name of the person making the offer to settle the account.
{DEBT_DESCRIPTION}
Describe the account or debt in sufficient detail to make certain that the
account or debt can be identified in case of a suit for collection.
{DELIVERY_ADDR1}
Give the first line of a two-line address to which the collateral is to be
delivered. This entry will appear as a completely separate paragraph in the
letter or notice. Two address lines are provided, if needed. You may skip
the second one by entering two returns. <RETURN><RETURN>

Last line of address to which the collateral is to be delivered. You may
skip this line by entering two returns. <RETURN><RETURN>

This entry will appear as a separate paragraph in the letter. It should set
out all of the reasons that the amount claimed due on the account or debt is
and has been in dispute.
{DRAWER_NAME}
This entry is for the name of the person signing the check as shown on the
signature line.
{DUE_DATE}
Enter the date the payment in default should have been made by the debtor.
{GUARANTEE_DATE}
This entry is for the date of the guarantee on which payment is sought for
the default of the note.
{INITIALS}
This entry is for the writer's initials and secretary's initials to identify
the parties responsible for issuing the letter, if desired. This line may
be skipped by entering 2 returns. <RETURN><RETURN>
{NOTE_DATE}
Enter the date of the Note which is in default.
{NOTE_MAKER(S)}
Enter the name or names of the person or persons who executed the note on
which demand for payment is being made as they appear on the note.
{NOTE_PRINCIPAL}
This entry is the numeric amount for which the note was originally made.
{PAYMENT_DUE}
Enter the numeric amount of the payment which is overdue.
{SALUTATION}
This line is for the salutation of the letter or notice. It may be skipped
by entering two returns: <RETURN><RETURN>
{SECURITY_DATE}
Enter the date of the security agreement or mortgage which was executed to
secure payment of the obligation which is in default.
{SETTLE_OFFER}
Enter the numeric amount for which payment is being offered in complete
settlement of the account or debt.

This entry is for the name of the person who will sign the letter or notice.
It will appear immediately below the space for his signature.
